Video/Materials Request

Per Executive Order No. 2021-1, individuals who have filed misconduct complaints may request the release of body-worn camera footage, in-car camera recordings, and Chicago Police Department reports by completing the form below. Such requests may only be submitted by the subject of the incident, their attorney, or their parent/legal guardian.

    A formal complaint must be filed with COPA for the incident in question, and a log number must be assigned. If you do not know your log number, please call (312) 746-3609 and select option 4 when prompted.
    A subject is the person who is the subject of alleged misconduct. The subject can also be the complainant if the complaint is self-reported. A case can have multiple subjects of the alleged misconduct is applied to multiple individuals.
  • The release of video and other materials under the process outlined in Executive Order No. 2021-1 is only eligible when:

    1. A formal complaint has been filed with the Civilian Office of Police Accountability with an associated log number; and
    2. The requestor is the subject of the misconduct complaint or is the subject’s parent/legal guardian or an attorney making the request on their behalf.

    If you are requesting video or other materials associated with an event or interaction with a Member of the Chicago Police Department that does not have a formal complaint or you are someone other than the subject, please visit Chicago.gov/publicrecords.
